
Program Description
The Enhanced Care Management (ECM) Program is designed to provide comprehensive, coordinated, and personalized care to children and young adults (ages 0 to 21), addressing their unique health and social needs. Offered in partnership with Health Net, this program is available to children who have Health Net as their primary insurance. The ECM program focuses on enhancing overall health outcomes and assisting families in navigating the healthcare system, particularly for children with complex medical needs.
Our team of Lead Care Managers (LCMs) and Community Health Workers (CHWs) collaborates to ensure that each child receives the proper care, resources, and support in a timely and efficient manner.
Key Services Provided:
- Comprehensive Care Coordination:
Our LCMs work closely with families to coordinate medical care, behavioral health services, and other necessary interventions. This includes scheduling appointments, ensuring follow-ups, facilitating specialist referrals, and managing care transitions. We ensure that the child’s healthcare needs are met across all areas of care. - Personalized Health Plans:
Every child receives a tailored care plan created in collaboration with healthcare providers and the child’s family. These plans may involve addressing chronic illnesses, providing preventive care, offering behavioral health support, and any other specialized services that may be required. - Health Education & Advocacy:
CHWs provide essential education to families, guiding them on managing chronic conditions, understanding medications, and promoting overall well-being. They advocate for families within the healthcare system to ensure children receive the care and support they need. - Support with Access to Healthcare Services:
Our team assists families in navigating the healthcare system by providing logistical support, such as scheduling appointments, arranging transportation, and overcoming any barriers that may prevent children from receiving necessary care. - Behavioral and Mental Health Support:
The ECM program places a strong focus on addressing mental and behavioral health needs. Our team works with families to connect children to mental health services, including therapy, counseling, and support for emotional or behavioral challenges. We aim to provide holistic care that encompasses both physical and mental health. - Community Resource Connections:
CHWs also connect families to valuable community resources, including housing support, food assistance, educational programs, and other essential services. By connecting children and families with essential community services, we ensure they are supported in all aspects of their lives.
Eligibility Criteria:
To be eligible for the ECM program, children and young adults must meet the following criteria:
- Health Net as Primary Insurance: The child must have Health Net as their primary health insurance.
- Mental Health Needs: The child must have mental health challenges that require support.
- Homelessness: This includes children who are couch surfing or sharing a home with multiple people.
- Multiple Hospitalizations or Chronic Illness: Children who have had frequent hospitalizations due to chronic illness are eligible for services.
- Welfare Involvement: Children who have an open DCFS (Department of Children and Family Services) case or are involved in the welfare system.
- Developmental Disabilities: Children with developmental disabilities that require special care or attention.
