SCHARP ICMS – Intensive Case Management System
Overview
LA County of Health Services Office of Diversion and Reentry (ODR) is partnering with community based providers to deliver reentry Case Management Services (R-ICMS) funded by SB 678 and Proposition 47. The goal of R-ICMS is to improve the health outcomes, reduce recidivism, and create lasting social safety nets for vulnerable justice-involved individuals.
Reentry Intensive Case Management Services
- Housing Support Services
- Employment and Education Services
- Referral and linkage to physical health, mental health, and substance abuse treatment services
- Stabilization needs (clothing, food, documentation, benefits, temporary shelter)
- Cognitive behavioral interventions
- And other supportive services based on client needs
Minimum Qualifications
- Active Medi-cal
- Jail/Arrest History
