Certified Community Behavioral Health Clinic/Planning Development Implementation (PDI)
SCHARP’s Certified Community Behavioral Health Clinic (CCBHC) is a SAMHSA-funded clinic in the Vermont Vista area of Central Los Angeles, SPA 6. SAMHSA’s CCBHC model is designed to ensure anyone who requests care for physical health screenings, linkage to physical health care, mental health or substance use, regardless of their ability to pay, place of residence, citizenship status, or age has timely access to coordinated comprehensive behavioral health care services.
SCHARP’s CCBHC multidisciplinary team includes experienced and compassionate health care navigators, case managers, peer specialists, a licensed Marriage and Family Therapist, and a Family Nurse Practitioner. In addition, SCHARP’s CCBHC program ensures people have access to crisis services that are available 24 hours a day, 7 days a week.
SCHARP’s CCBHC clinic offers a wide array of intensive targeted case management services, comprehensive care coordination, and high-touch navigation of physical and mental health services across multiple providers, social services, and other systems consumers are involved in. Our CCBHC commitment is to remove the systemic barriers that prevent people in our community from gaining access to high-quality, person-centered health and mental health care coordination and navigation.
