Program Description

Program Goal:

At Southern California Health & Rehabilitation Program (SCHARP), our SUD Outpatient Treatment Program is tailored to support individuals navigating the challenges of substance use and co-occurring disorders, providing a balance between independence and structured recovery support. These services are essential for those learning to manage their emotions and address mental health and substance use issues constructively and effectively. Our Outpatient Programs provides up to six-months of treatment that involves regular and evolving therapy sessions, where patients receive evidence-based approaches which are unique to their needs and aspired goals. These methods are specifically designed to target behavioral, cognitive, and emotional aspects associated with addiction and substance use, offering personalized solutions for recovery.

The goals and objectives of SCHARP’s Outpatient Treatment (OP) and Early Intervention Services for Adults center around providing educational insights into substance use, dual diagnosis, understanding the consequences of substance abuse, and developing skills for relapse prevention, identifying potential triggers, and building a robust support network. OP treatments focus on establishing personalized treatment goals that reflect each patient’s strengths, recovery hurdles, and presenting needs.

SCHARP’s Outpatient SUD Level of Care:

  • ASAM 0.5 – Early Intervention Services for Young Adults
  • ASAM 1.0 – Outpatient Treatment Program

SCHARP will meet the following program goals within each fiscal year:

  1. Decrease morbidity and mortality from substance use and mental health disorders.
  2. Enhance the health and well-being of our patients.
  3. Provide community resources for substance use disorders and mental health.
  4. Integrate SUD and mental health care with physical health treatment.
  5. Improve access, initiation, engagement, and retention rates.
  6. Support patients in developing active recovery programs.

The program objectives (each noted in the table below) are linked with one or more specific goals and describe the specific result to be achieved. Each objective is designed with the “SMART” attributes in mind. “SMART” attributes are used to develop a clearly defined objective. “SMART” objectives are:

  • Specific – including the “who,” “what,” and “where” of the objective.
  • Measurable – focus on “how much” change is expected.
  • Achievable – realistic given program resources.
  • Relevant – relate directly to program/activity goals.
  • Time-bound – indicates “when” the objective will be achieved.
